Support Coordinator


Support coordinators work with clients with developmental disabilities, and people identified by the client, such as family, to develop a written individualized plan of services. The support coordinator then links, and monitors the client with the appropriate services, and continually follows-up to ensure the client is receiving the best care. A support coordinator will also provide assistance with access to entitlements, such as Medicaid.

How much will services cost?
Fees for service are based on your ability to pay, and your health insurance coverage. Consumer Services, Inc. accepts most private insurance, and will verify coverage and co-pays upon initial contact.
